1. For information and guidance of all concerned, enclosed in Division Memorandum No. 412 s. 2018, stipulated therein is the schedule of the submission and screening of Athletes and Coaches Documents on November 16-23, 2018 in your respective schools/districts.

2. In line with this, the members of the Division Screening and Accreditation Committee (DSAC) of both Elementary and Secondary Schools are mandated to evaluate and verify the authenticity of the documents of athletes and coaches of the ten (10) District delegation and eight (8) secondary schools including the two (2) private schools in your respective jurisdiction.

3. Atached herewith is the list of the DSAC members both elementary and secondary to wit:

Kindly submit the complete set of documents with transmittal to Rosita B. Lim for the secondary level and Ma. Cecilia C. Llever for the Elementary level.

5. The “No Documents, No Play” policy will be strictly observed. 6.Immidiate dissemination of this Memorandum is desired.
