                                   REITERATION OF THE SUBMISSION OF REPORTS ON DEPED CHILD
                                                                 PROTECTION POLICY FOR SY 2017-2018

TO:           Catbalogan I, II, IV and V Public

Schools District Supervisors and School Heads, and Private
School Heads

1.     Pursuant to DepEd Memorandum (DM) No. 114, s. 2017 or the “Reiteration of the

Reportorial Obligation of Schools, Schools Division Offices (SDOs), and Regional Offices (ROs) pursuant to DepEd Order (DO) no. 20, s. 2012, DO No. 55, s 2013, and DO No. 18, s. 2015,” all public and provate schools are directed to accomplish the required reports using the prescribed templates and submit the same to their respective Division Offices for consolidation.

2.    The Consolidated Division Reports in proper formats shall likewise be submitted to the

Regional Offices for consolidation on or before June 30,2018.

3.   To reiterate, the abovementioned repots and submissions for schools are the

following;

a. Report on Incidents of Bullying (Enclosure 1A of DM 59, s. 2015)

b. Reports on Incidents of Child Abuse (Enclosure 1B of DM 59, s. 2015)

c. Reports on Cases involving CAR and CICL in Schools (Appendices’D and F of
      D018, s. 2015)

d. Localized Child Protection Policy and Anti-Bullying Policy

e. Child Protection Committee

4.    Catbalogan III District has complied 100% with the repotorial requirements while other

districts and private schools have lacking submissions to zero and nearly zero compliance.

5.   Enclosures 1A and 1B and Appendices D and F abovementioned are hereto attached

for easy reference.
