REQUIREMENTS FOR CORRECTION OF PERSONAL INFORMATION IN THE SCHOOL
                                                             RECORDS

To:       Schools Division Superintendents

All Others Concerned

1.             This Office hereby informs the field that the requirements for correction of personal information in the school records that must be complied with by the concerned applicants are the following, to wit

a. Duly filled-out Application Form

b. Original Copy of Certificate of Live Birth issued by the Philippine Statistics
      Authority

c.     Affidavit of Two Disinterested Persons

d. Certified Photocopy of Form 137

e. Photocopy of a valid ID

f.      Special Power of Attorney (if request is being made through a representative,

except to a father or mother if the applicant is still a minor)

2.              Immediate dissemination of and compliance with this memorandum are desired.
