1.     In reference to Memorandum Circular No. 087 s. 2018 dated April 16, 2018 issued by the Office

of the City Mayor, re: “Submission of Annual Accomplishment Report 2017, First Quarter Accomplishment Report 2018 and Local Nutrition Action Plan PPAN 2017-2022”, this division requires all Field Public Schools District Supervisors to submit a district consolidated report on ANNUAL ACCOMPLISHMENT REPORT FOR 2017, FIRST QUARTER

ACCOMPLISHMENT REPORT FOR 2018 and LOCAL NUTRITION ACTION PLAN PPAN 2017-2022 to School Health and NutiCtion Unit on or before April 20, 2018. Attention: Sherrydale Queen H. Uy, City Nutrition Coordinator for division consolidation.

2.     Relative to the said memorandum, accomplishment reports on Programs and Projects related to

Nutrition Education which are reflected in the 2017 AIPs of all schools per district of the Division shall be included in the attached template. To wit:

• Weekly Iron Folic Acid Supplementation to learners rom grade 7-10

• Gulayan sa Paaralan Program

• School Based Feeding Program

• Wash in School (WinS) Program

• Conduct of School Based Immunization to Grade 1 and Grade 7 learners

• Other school-based Programs and Activities conducted related to Nutrition
      Education

1.  Accomplishment Report for First Quarter of 2018 2. Annual Report Accomplishment Report for 2017 3. Local Nutrition Action Plan PPAN 2017-2022

For Elementary School Districts I, II, III, IV & V please submit your respective accomplishment report and PPAN to Ms. Sherrydale Queen Uy, City Schools Division Nutrition Coordinator for consolidation.

Please be informed further, as agreed during the meeting that starting 2018 Accomplishment Report for Nutrition Program implementation of your respective ofice/sector shall be submited quarterly on or before the 7th day of the following month for a particular quater. For other ofice/sector, please submit the above mentioned documents to Elizabeth U, Veesco, CNC Secretary at the Office of the City Mayor.

Deadline for submission of these documents is on or before Apil 30,2018.
