SUBJECT: ANNUAL PHYSICAL EXAMINATION OF
DEPED EMPLOYEES
(TEACHING
AND NON-TEACHING PERSONNEL)
DATE: April 20,2018
1. Pursuant to DepEd memorandum No. 22, s. 2015 entitled Annual Physical Examination of
DepEd Employees (Teaching and Non-Teaching Personnel), as a manifestation of concern by this office on the health status of its employees, all teaching and non-teaching are required to undergo Annual Physical Examination (APE) including Dental Examination in
compliance with the Civil Service Commission Memorandum Circular No. 17, s. 1989.
2. In this regard, all teaching and non-teaching personnel are required to bring their CS form 86
on their scheduled date and present it to School Health and nutrition Unit to get a copy of the medical certiicate and laboratory request for the following:
• Chest X-ray Result except for pregnant women
(within 6 months before the
respective scheduled date)
• Urinalysis (within the week pior to the respect schedule date)
3. Further, Medical certificates must only be signed by any Government Medical Doctor.
4. The following schedule should be strictly complied with accordingly:

5. The teacher’s annual health clearance shall be based on the filled out and duly accomplished
CS Form 86 and Medical Certificate (Health Examination Record).
6. Immediate dissemination and compliance of this Memorandum is desired.
