SUBJECT Submission of Original Copies of Certificates of
Participation/Recognition

In line with the Electronic Computerization Office (ECO) System, all Personnel of the Division of Catbalogan City, Samar, this Office hereby requests for the submission of original copy/ies of Certificates of Participation & Certificates of Recognition in electronic copies (e-copies).
In this connection, the Certificates should not have been used for past promotions and have been issued for the past five (5) years and should have the following number of days:
• At least 3 days minimum for division seminars;
• At least 1 day for national seminars.
3. The electronic copies should be sent by the School ICT Coordinator in folder file to the District
ICT Coordinator, the latter will be the one responsible for sending the aforementioned files in folder to the Division Records Officer thru the email ad: fe.samonte001(5) deped,gov.ph.

4. Relative thereto, worthy to mention that these electronic documents will be utilize as
reference on future pertinent merit and selection process, non-submission of the same will
be construed as non-appreciation for purposes of evaluation thereof.
5. Deadline of submission is on March 15, 2018.
6. Strict compliance of this Memorandum is enjoined.
