1. The Department of Education (DepEd) in partnership with the Department of Health (DOH) through
the School Health and Nutrition Unit of School Governance and Operations Division (SGOD) shall
implement the Second Round of National School Deworming Month for the whole month of January from Kinder to Grade XII learners.
2. This activity aims to cover all school age learners enrolled in public elementary and secondary school.
Private schools that are interested to participate are advised to coordinate with the School Health and Nutrition Unit.
3. The following are the schedules and the Nurses in charge in your respective schools:

4. School Heads are requested to secure all deworming forms together with the parent’s consent and
coordinate with the assigned nurse for inquiries and easy implementation of the program.
5. A Consolidated Deworming Report shall be submitted by the School Heads on or before January 29,
2018.
6. Immediate dissemination of this memorandum is earnestly desired.
