SUBMISSION OF APPLICATION FOR TRANSFER OF
TEACHERS
WITHIN

Pursuant to DepEd Order No. 22, s. 2013 re:
“Revised Guidelines on the Transfer of Teachers from one station to another” and in order to have a
pool of applicants for transfer for SY 2018-
2019, all applications for transfer from one
school/station to another should be submitted to this
office c/o Personnel Unit on or before May 15, 2018. Failure to submit on the
due date will mean a waiver on the part of the teacher
concerned.
Applicants for transfer shall submit 2 sets/folders with the following documents:
a. Letter of intent to transfer indicating at least 3 preferred schools to be transferred in and
the reason(s) for transfer.
b. Personal Data Sheet (PDS revised 2017)
c. Service Record
d. Performance Ratings for the last three (3) rating periods.
e. Supporting documents for the reason(s) stated on the letter of intent to transfer.
This pool of applicants for transfer will be considered in
the deployment of Kindergarten to
Grade 10 teachers for School Year 2018-2019.
Therefore, those whose requests were not yet
granted are advised to resubmit their letter of intent if they are still interested to be transferred.
Widest and immediate dissemination of this Memorandum is
desired.
