Submission of the Partial January to February WIFA Accomplishment Report

1.     In line with the implementation of the 2nd Round Weekly Iron-Folk Acid (WIFA)

Supplementation for Female Adolescent Learners in Public High School program, the School Health and Nutrition Unit of this division will be gathering a partial Accomplishment Repot for the months of January to February 2018.

2. Further, the Field Public Schools Division Supervisors (PSDSs) for the Secondary
      Level and ALS Supervisor are requested to submit the Reporting Form 3D on or
      before March 16,2018 to the abovementioned Unit. Attention: Sherrydale Queen

H. Uy, WIFA Coordinator.

3.     Attached is the sample format to be used as your reference.

4. Immediate disseminaion of this Memorandum is desired.
