SUBJECT:                        SUBMISSION OF NUTRITIONAL STATUS REPORT (ENDLINE) SY 2017-2018

1. The Department of Education      (DepEd) together with the National Nutrition Council                                                                             (NNC)

continuously assesses the Nutritional Level of all Filipino learners for the implementation of the appropriate interventions. Nutritional Status Report has evolved to become the key source of data not just on nutrition-related information but on health matters as well.

2. In line with this, all public and private elementary and secondary school heads are required to submit
        their Consolidated Nutritional Status Report (Endiline) SY 2017-2018 to their respective Public School
        District Supervisors on or before March 23, 2018. Subsequently the same will be submitted by the
        latter to School Health and Nutrition Unit with attached transmittal attention: Chatelaine M.
        Macopia.

3.     Immediate dissemination and compliance of this Memorandum is desired.
